New Mandated Reporting Training and Form Completion

All Roosevelt University counselor candidates are required to complete an online training course for mandated reporters by the time of the first meeting of their practicum or internship.  This requirement applies to both school and clinical mental health counselor candidates.

The required training includes:

A pre-training assessment (13 multiple-choice questions)
60-90 minutes of self-paced interactive training
A post-training assessment (13 multiple-choice questions)
A Certificate of Completion (which must be printed at time of completion)

Upon completion of the training, turn in your Certificate of Completion and a signed Acknowledgement of Mandated Reporter Status Form to your university instructor. Please staple your forms together. Your university instructor will forward your signed and stapled forms to the Office of Field Placement, for batch mailing to the Office of the Director of DCFS, as required by state rule.  A copy of all submitted forms will be placed in your advising file.

University instructors should turn in ALL signed and stapled forms at one time to either Monica Kielar on the Schaumburg campus, or Dana Blanton on the Chicago campus.

Beginning in Spring 2011, this required training will take place within CHS 426, the Pre-practicum.
